Simple Steps To A Healthier Life website, on Aetna's Terms of Use, you will have the option to share or not share your data from your Health Assessment questionnaire with Aetna or not on Aetna's Terms of Use page. If you
choose not to share your data from your Health Assessment questionnaire with Aetna, you will still have access to the following online programs: Simple Step Programs, Healthy Aging, Healthier Diet, Get in Shape, Healthy Heart, Weight Loss, Stress Relief, Smoking Cessation, Alcohol Awareness, Cancer Fighting, and Disease Fighting, based upon your Health Assessment questionnaire score, but not to any of the following outreach programs (someone will personally contact you): Weight Management, Wellness Counseling or Quit Tobacco. If you are eligible for the additional outreach programs and you chose not to share your data from the Health Assessment questionnaire with Aetna, but now wish to access the additional outreach programs (if eligible), you may log back in to retake the Health Assessment questionnaire and uncheck the "do not share" box on the "My Profile" page. If you initially chose to share your data from your Health Assessment questionnaire with Aetna, you may later choose to not share by checking the box on the "My Profile" page, however, your new choice not to share your data with Aetna will not revoke your prior authorization to share data with Aetna from your prior Health Assessment questionnaire.
If you decline to agree to Aetna's Terms of Use on the Aetna
Simple Steps To A Healthier Life website, you will not have access to take the Health Assessment questionnaire nor any of the programs listed in the paragraph immediately above for which you may have been eligible.
When you register to take the Health Assessment questionnaire on Aetna's
Simple Steps To A Healthier Life web site, Aetna will require either your Aetna ID number on your Aetna Medical Card or if you do not have an Aetna Medical Card, then your social security number. Your social security number will be used for verification and validation purposes only and will not be disclosed to any third party.
Aetna will keep your personal health information confidential and NOT disclose your and/or your family member's personal health information to ConocoPhillips. Only aggregated, de-identified information will be provided to ConocoPhillips to show site usage information such as the number of registrants, site visits, and Health Assessment questionnaire completions, as well as the number of registrants participating in particular health management programs, what percentage of ConocoPhillips' population are at risk of certain health conditions, and information on how certain illnesses impact a company's workplace productivity.
Retirees may also be eligible to take the Health Assessment questionnaire.
